What the Risk Report Checks
- Operating Authority
- Confirms the carrier holds active FMCSA authority to haul freight. Inactive or revoked authority is an automatic fail.
- Insurance on File
- Verifies whether the carrier has active insurance on file with FMCSA via MC authority status.
- Safety Rating
- Official FMCSA safety rating: Satisfactory, Conditional, or Unsatisfactory — based on compliance reviews.
- OOS Violation Rate
- Percentage of inspections that resulted in an out-of-service order. High OOS rates indicate chronic safety problems.
- Crash Rate
- Recordable crashes per million miles driven, as reported to FMCSA by state enforcement agencies.
- Overall Risk Grade
- A single A–F letter grade combining all factors so your team can make a fast, consistent onboarding decision.

How the Risk Grade Is Calculated
The risk grade combines four factors from public FMCSA data. Two are pass/fail gates — if either fails, the carrier receives an automatic F regardless of other scores:
- Operating authority — must be Active
- Insurance on file — MC authority must be Active (FMCSA requires insurance to maintain active MC status)
If the carrier passes both gates, the remaining score is weighted: OOS rate (40%), safety rating (35%), and crash rate (25%). Grades run A through F with A representing the lowest risk.
